Hey I had the exact exact same issue (on my Yamaha APX500II) and found this post online!
I was having this issue for weeks, then I tried a different cable after reading your post. The cable I was originally using was either balanced or stereo 1/4 inch, because it had an extra ring. It was also one of the slightly skinnier cables.
I switched to a more standard instrument cable, no extra rings, and a normal width, and the problem went away.
I think that the extra ring on the cable was messing with the Yamaha's ability to detect the cable being inserted properly.
Anyways, try a standard instrument cable. Hope this helps!
