I found two videos from a German keyboard player. He couldn't use his left hand for a while after shoulder surgery. The videos are in German, but I think the pictures alone are enough to explain.
Fingering Type "Full Keyboard":

(You can play the melody in one voice or in two voices. The accompaniment only changes chords when at least three keys are played simultaneously.)
MIDI foot pedal:

The following video shows how to connect and set up a MIDI keyboard. It should work the same with a MIDI foot pedal. This video is also in German, but you might find it helpful to turn on generated English subtitles.
